The answer actually comes to you near the end of the game. The Snake Hearts or Serpent Viscera, comes in two forms. A Dried Serpent Viscera and Fresh Serpent Viscera. Both are required to receive the optional ending of the game, so be sure to find them along the way. Once you have them, you need to hold onto them until you’ve reached the Inner Sanctum, an area that is unlocked after beating the Folding Screen Monkeys.
Even after you beat them, you aren’t ready to use them. At the Inner Sanctum you meet the Divine Child of Rejuvenation. She will offer you Rice, which is useful in a number of ways. After accepting and consuming the White Rice a few times, and completing certain objectives she will request the Snake Hearts from you. Handing them over will put you on the path to Sekiro’s optional and better ending.
